Important Notice: Submission Documents for Fall 2025 Graduate School Thesis Evaluation Application

Guidelines for submitting documents for the 2025 Fall semester General Graduate School thesis evaluation application
(We are providing additional guidance as there have been many inquiries regarding submission documents.)

• Thesis evaluation application document submission period: October 20 (Mon) – October 24 (Fri), 2025, by 17:00

a. Required submission documents: Submit the thesis for evaluation and related documents to the respective college administrative office

1. Recommendation letter for thesis committee members and list of committee members must be prepared by the academic advisor
2. Administrative confirmation on the application form will be verified by the college administrative office
3. One copy of the thesis for evaluation must be delivered directly to the advisor, and the remaining copies must be delivered to the committee members
   (A “direct submission confirmation form” must be submitted to the College of Health Science Administration Team – see attached file)
4. The thesis evaluation application form must be printed after online submission
5. After submitting the application form and all related documents to the College of Health Science Administration Team, further instructions will be provided by the administration team
